Imagine sitting in a room with a pile of medical records to review each and every day. Imagine that you have to make a medical underwriting decision based on your company’s criteria regarding height, weight, medical history, complaints, diagnosis, medications, compliance of patient and combination of health concerns. Your job is to do the underwriter's job and not leave anything open-ended. The better the story, the better the rating, the better the price...
Doctor visits
Name, address & phone number of each doctor. Date of visit, reason for visit, diagnosis, follow ups, medications prescribed and compliance with medication and date and reason stopped.

Lab Report
Any lab reports within one year old is good to give your insurance agent so there is no surprises.

Dr. Notes
If you have medical history that needs to be verified, these notes will be requested & reviewed.

Medications prescribed
Doctor’s very often will prescribe something. Their notes will disclose this.

Medications taken
Very often people fill prescriptions but don’t comply with the instructions. Therefore, the insurance company does not know if you even needed medication or what the result was.

Medications not taken or discontinued
Unless you report back to your doctor and they put in in your notes, your underwriter will assume that you completed the medication as prescribed.

Conclusion
By not properly disclosing the above at application time, your underwriter may determine that you are trying to hide something and not give you the rating you deserve.  You may end up paying more for your policy as a result. Many consumers. Overpay for life insurance because.  They did not get the best underwriting consideration due to inaccurate, incomplete and misleading doctor’s notes.
